Running a Pharmacy in India: A Complete Guide

Drug licenses, Schedule H/H1/X controlled substances, GST on medicines, and FEFO stock management — everything your pharmacy software must handle

Drug License Types: Forms 20, 21, 20B & 21B

Under the Drugs and Cosmetics Act, 1940, all pharmacies in India must obtain the appropriate drug license before operating. The correct form depends on your business type:

License Form Type Who Needs It Key Requirement
Form 20 Retail — Allopathic Standard retail pharmacy stores Registered Pharmacist on premises during open hours
Form 21 Retail — Schedule H1/X Stores selling controlled substances Separate register required for Schedule X drugs
Form 20B Wholesale — Allopathic Medicine distributors & stockists Qualified person with degree/diploma in pharmacy
Form 21B Wholesale — Schedule H1/X Wholesale stockists with controlled substances Strict register maintenance, audit-ready records

Licenses are issued by the State Drug Controller / Licensing Authority. Renewal is required every 5 years. Operating without a valid license carries heavy penalties under Section 27 of the Act.

Schedule H, H1 & X: Controlled Substance Handling

The Drugs and Cosmetics Act classifies medicines by their risk profile. Your pharmacy software must track these three schedules differently:

Schedule H — Prescription Only

Cannot be sold without a valid prescription. Prescription must be retained for 2 years. Printed label must carry the "Rx" symbol.

Examples: Antibiotics (amoxicillin, ciprofloxacin), antihypertensives, metformin, omeprazole

Schedule H1 — High-Risk Rx

Stricter controls than H. Pharmacist must countersign. Sales must be entered in a register with patient name, address, and prescribing doctor details.

Examples: 3rd-generation antibiotics (cefixime), anti-TB drugs (rifampicin), antiretrovirals

Schedule X — Psychotropic/Narcotic

Most restrictive class. Separate register mandatory, NDPS Act compliance required, subject to periodic government inspection.

Examples: Tramadol, codeine, morphine, benzodiazepines (alprazolam, clonazepam)

GST Rates on Medicines & Medical Devices

Pharmacy billing spans four GST slabs. Your software must automatically apply the correct rate per product to ensure accurate GSTR filing:

0%

Nil Rate

Life-saving medicines (government notified), oral rehydration salts, blood products, contraceptives

5%

5% Rate

Vaccines, diagnostic kits, Ayurvedic medicines, insulin, diagnostic reagents

12%

12% Rate

Most formulated prescription medicines, non-electric medical devices (BP monitors, glucometers)

18%

18% Rate

Surgical instruments, dental equipment, X-ray machines, hospital furniture

FEFO vs FIFO: Why Pharmacies Must Use FEFO

Inventory rotation methods differ across industries. For pharmacies, FEFO (First Expired, First Out) is not just best practice — it is a CDSCO compliance requirement:

FIFO — First In, First Out

The oldest stock (by purchase date) is sold first, regardless of expiry date. Standard in retail, restaurants, and general stores.

⚠️ Risk for pharmacies: A batch purchased later may expire earlier than an older batch — FIFO would sell the longer-dated stock first, leaving the soon-to-expire batch behind.

FEFO — First Expired, First Out

The batch with the earliest expiry date is sold first, regardless of when it was purchased. Mandated by CDSCO Good Distribution Practices (GDP) guidelines.

✅ OnePlaceHQ automatically suggests the correct FEFO batch at billing time and alerts you 30–90 days before expiry.

Frequently Asked Questions

Does it support batch-wise inventory?

Yes! OnePlaceHQ supports batch-wise inventory management with individual expiry dates, purchase prices, and MRP tracking for each batch.

Can I get expiry alerts before medicines expire?

Absolutely! You can set custom expiry alert periods (30, 60, 90 days). Get daily reports of expiring stock and FEFO suggestions for selling.

Is GST filing easy with this software?

Yes, 100% GST compliant. All invoices include proper GST breakdown. You can export GSTR-1 and GSTR-3B data directly from the system.

Can I integrate with my hospital's system?

Yes! If you're using OnePlaceHQ Hospital Management, the pharmacy module integrates seamlessly. Prescriptions flow directly, and billing is linked.

Does the software track Schedule H, H1, and Schedule X medicines with separate registers?

Yes! OnePlaceHQ maintains separate compliance registers for Schedule H (prescription-only), Schedule H1 (high-risk), and Schedule X (psychotropic/narcotic) medicines as required by the Drugs and Cosmetics Act. Each sale of a controlled substance is logged with prescription details, patient information, and pharmacist countersignature — keeping you audit-ready.

Can I create and track purchase orders to medicine suppliers?

Yes! Generate purchase orders directly from the system when stock falls below reorder levels. Track PO status from raised to received, receive stock against POs with batch-wise entry, and reconcile supplier invoices automatically to prevent billing errors.

How does the system handle different GST rates for different medicines in the same bill?

Each medicine in the system is pre-configured with its correct GST rate (0%, 5%, 12%, or 18%). When you add multiple medicines to a bill, the system automatically applies the right rate per line item. The invoice clearly shows the GST breakdown per slab, and your GSTR-1 report correctly separates revenue by rate — no manual calculation needed.
