Drug License Types: Forms 20, 21, 20B & 21B
Under the Drugs and Cosmetics Act, 1940, all pharmacies in India must obtain the appropriate drug license before operating. The correct form depends on your business type:

Licenses are issued by the State Drug Controller / Licensing Authority. Renewal is required every 5 years. Operating without a valid license carries heavy penalties under Section 27 of the Act.
Schedule H, H1 & X: Controlled Substance Handling
The Drugs and Cosmetics Act classifies medicines by their risk profile. Your pharmacy software must track these three schedules differently:
Schedule H — Prescription Only
Cannot be sold without a valid prescription. Prescription must be retained for 2 years. Printed label must carry the "Rx" symbol.
Examples: Antibiotics (amoxicillin, ciprofloxacin), antihypertensives, metformin, omeprazole
Schedule H1 — High-Risk Rx
Stricter controls than H. Pharmacist must countersign. Sales must be entered in a register with patient name, address, and prescribing doctor details.
Examples: 3rd-generation antibiotics (cefixime), anti-TB drugs (rifampicin), antiretrovirals
Schedule X — Psychotropic/Narcotic
Most restrictive class. Separate register mandatory, NDPS Act compliance required, subject to periodic government inspection.
Examples: Tramadol, codeine, morphine, benzodiazepines (alprazolam, clonazepam)
GST Rates on Medicines & Medical Devices
Pharmacy billing spans four GST slabs. Your software must automatically apply the correct rate per product to ensure accurate GSTR filing:
Nil Rate
Life-saving medicines (government notified), oral rehydration salts, blood products, contraceptives
5% Rate
Vaccines, diagnostic kits, Ayurvedic medicines, insulin, diagnostic reagents
12% Rate

18% Rate

FEFO vs FIFO: Why Pharmacies Must Use FEFO
Inventory rotation methods differ across industries. For pharmacies, FEFO (First Expired, First Out) is not just best practice — it is a CDSCO compliance requirement:
FIFO — First In, First Out
The oldest stock (by purchase date) is sold first, regardless of expiry date. Standard in retail, restaurants, and general stores.
⚠️ Risk for pharmacies: A batch purchased later may expire earlier than an older batch — FIFO would sell the longer-dated stock first, leaving the soon-to-expire batch behind.
FEFO — First Expired, First Out
The batch with the earliest expiry date is sold first, regardless of when it was purchased. Mandated by CDSCO Good Distribution Practices (GDP) guidelines.
