Running a Clinic in India: A Digital Guide
Everything Indian doctors need to know about digital health regulations, OPD queue management, and patient data compliance
India's Healthcare Landscape
India has 13.86 lakh registered allopathic doctors (as of November 2024, per NMC), but an estimated 5-8 lakh of them run solo or small 2-3 doctor clinics. Despite India's booming digital economy, the vast majority of these clinics still run on paper registers, handwritten prescriptions, and WhatsApp groups for appointment coordination. The digital health market is projected to grow from USD 14.5 billion in 2024 to USD 107 billion by 2033 — yet adoption at the clinic level remains slow. Post-COVID, patients increasingly expect digital experiences: prescriptions on WhatsApp, appointment reminders, and accessible records. The software gap is widening.

OPD Queue Management: Why India Is Different
Indian OPDs are fundamentally different from Western outpatient clinics. An estimated 70% of patient visits to Indian clinics are unscheduled walk-ins — patients who arrive without an appointment and wait in a physical queue. This is driven by low digital booking adoption among older demographics, the cultural expectation of "just show up", and the absence of booking tools at most clinics. The right software must handle walk-ins gracefully, not eliminate them.
| Queue Mode | How It Works | Best For | Key Benefit |
|---|---|---|---|
| Token (Walk-in) | Sequential tokens issued at reception. FIFO queue, daily reset. | High-volume GP clinics, morning OPDs | Handles any volume with no pre-booking required |
| Slot (Appointment) | Fixed 15/20/30-min slots booked in advance. No double-booking. | Specialists, procedure-based consultations | Predictable wait times, zero overruns |
| Hybrid | Reserved slots for bookings + overflow tokens for walk-ins. | Multi-doctor clinics, mixed practice types | Best of both — planned and spontaneous visits |
WhatsApp queue notifications — "2 tokens away, please proceed to reception" — reduce wait anxiety and improve patient experience without requiring any app install. OnePlaceHQ sends these automatically via the WhatsApp Business API.
NMC 2023 Prescription Mandates: What Every Doctor Must Know
The National Medical Commission's Registered Medical Practitioner (Professional Conduct) Regulations 2023 introduced the most significant prescription reform in India's history. The key mandate: all prescriptions must use generic drug names (International Nonproprietary Names, or INN). Violators face a 30-day suspension for a first offence. A Punjab & Haryana High Court ruling in August 2025 reaffirmed that legible medical prescriptions are a Right to Health under Article 21 of the Constitution.
What is MANDATORY on every Rx
- ✓Generic (INN) drug names — "Amoxicillin 500mg", not "Augmentin"
- ✓Doctor's Medical Council registration number on every Rx
- ✓Full dosage details: drug, dose, frequency, route, duration
- ✓Legible, preferably printed format (not handwritten)
- ✓Records retained minimum 3 years (NMC requirement)
What OnePlaceHQ auto-handles
- ✓Generic drug database — only INN names selectable during consultation
- ✓Registration number on every Rx (set once in doctor profile)
- ✓Structured dosage format enforced at input
- ✓Auto-generated PDF sent to patient via WhatsApp after consultation
- ✓3-year digital record retention, searchable by patient or date
ABDM & ABHA: India's National Digital Health Infrastructure
The Ayushman Bharat Digital Mission (ABDM) is the Government of India's national framework for a connected digital health ecosystem. At its core is the ABHA (Ayushman Bharat Health Account) — a unique 14-digit health ID linked to a person's medical history. As of January 2026, 84.79 crore ABHA IDs have been created, with 82.69 crore health records linked. The Health Facility Registry (HFR) now has over 4.18 lakh registered facilities, and the Healthcare Professionals Registry (HPR) has 7.38 lakh registered practitioners.
ABDM is currently voluntary for private clinics — but the adoption trajectory is unmistakable. Government hospitals and empanelled facilities are already mandated to comply. The FHIR (Fast Healthcare Interoperability Resources) standard underpins record exchange: a patient with an ABHA ID can share their medical history from any compliant facility with any other. Clinics that are not ABDM-ready today will face a costly migration later — especially as health insurance claims and Ayushman Bharat PMJAY empanelment increasingly require digital records.
What OnePlaceHQ stores: ABHA ID per patient, FHIR-compatible health records (SOAP notes, vitals, prescriptions, lab results), and Health Facility Registry identifiers — so your clinic is ABDM-ready without additional steps when it becomes mandatory.
DPDPA 2023: India's Data Protection Law and Your Clinic
The Digital Personal Data Protection Act 2023 is India's first comprehensive data protection legislation. Health data is classified as "sensitive personal data" — meaning clinics face the highest tier of obligations. The DPDP Rules 2025 require that patient consent records be retained for 7 years, and CERT-In mandates audit log retention for a minimum of 180 days.
| DPDPA Obligation | What It Means for Your Clinic | OnePlaceHQ Approach |
|---|---|---|
| Explicit Consent | Obtain written consent before collecting patient data | Consent captured at registration, timestamped |
| Consent Records | Retain consent records for 7 years (DPDP Rules 2025) | Automatically retained, auditable |
| Patient Rights | Patients can request data access, corrections, erasure | Doctor can export or delete records on request |
| Audit Trail | Retain access logs minimum 180 days (CERT-In) | All access logged, tamper-proof |
| Data Security | AES-256 encryption, TLS 1.2+ mandatory for health data | End-to-end encryption, row-level tenant isolation |
Why Clinics Go Digital: Common Challenges
The business case for clinic management software in India is not about features — it is about solving specific daily frustrations that erode both patient experience and revenue.
No-shows (25-38%)
Patients forget appointments or don't show up. WhatsApp reminders sent 24 hours before can reduce no-shows to 10-12% — recovering Rs 500-1,500 per missed consultation per day.
Illegible Prescriptions
Handwritten prescriptions cause dispensing errors and now violate NMC 2023 standards. Printed digital Rx eliminates this risk and auto-delivers to the patient's WhatsApp immediately after consultation.
Paper Record Retrieval
Locating a patient's old file during a consultation takes minutes of distraction. Digital records are searchable by patient name, phone, or ABHA ID in under 2 seconds.
Billing Errors & Leakage
Manual fee calculations for multi-procedure visits lead to underbilling or patient disputes. Per-doctor fee schedules with first-visit vs. follow-up pricing handle this automatically. See our GST Billing Guide for healthcare billing compliance.
Lost Follow-up Patients
Patients who need a follow-up "in 2 weeks" are rarely reminded. Automated WhatsApp follow-up reminders bring patients back — improving health outcomes and clinic revenue simultaneously.
Multi-doctor Coordination
In multi-doctor clinics, managing separate queues, fee structures, and schedules per doctor requires software — not whiteboards and notebooks. Each doctor can have their own mode, calendar, and patient list.
Frequently Asked Questions
Is patient data secure?
Yes. All patient data is encrypted at rest (AES-256) and in transit (TLS 1.2+). Each clinic's data is isolated using row-level security — no other clinic can ever access your records. We comply with DPDPA 2023 (India's data protection law) with built-in consent management and audit trails. We never sell or share patient data with anyone.
Can multiple doctors use the system?
Yes! Add as many doctors as you need. Each doctor gets their own login, appointment calendar, fee schedule, and queue. You can set different scheduling modes per doctor — one on token-based walk-ins, another on fixed slot appointments.
What about NMC prescription compliance?
OnePlaceHQ prescriptions are fully NMC 2023 compliant. All drugs use generic (INN) names from our curated database, your Medical Council registration number appears on every Rx, and dosage includes drug, dose, frequency, route, and duration as mandated. Prescriptions are auto-generated as PDFs and sent to patients via WhatsApp.
What is ABDM and do I need it?
ABDM (Ayushman Bharat Digital Mission) is India's national digital health framework. Over 84 crore ABHA health IDs have been created. While currently voluntary, it's rapidly becoming the standard — 62% of registered facilities still aren't using compliant software. OnePlaceHQ stores ABHA IDs and generates FHIR-compatible health records, so you're ready when it becomes mandatory.
How is this different from Practo or HealthPlix?
Three key differences: (1) WhatsApp-first — we use WhatsApp for all patient communication, not SMS. (2) Token + Slot hybrid queue — we handle both walk-in tokens and scheduled appointments, which no competitor does well. (3) Your data stays yours — unlike HealthPlix which offers free EMR but monetizes doctor and patient data through pharma analytics, we have a transparent model with zero data selling.
How much does OnePlaceHQ Hospital cost?
OnePlaceHQ Hospital is priced for Indian clinics — affordable for solo practitioners and scalable for multi-doctor setups. Contact us on WhatsApp for current pricing and to discuss your clinic's specific needs. We offer a free onboarding period so you can start with zero risk and evaluate the software with real patients before committing.
Can I migrate from paper records or existing software?
Yes. We help you migrate patient records from paper or from other software. For paper records, we can import a CSV of patient names, phone numbers, and basic history. You can also start fresh and build records digitally as patients visit — most clinics find this easier than bulk import. For existing software (HealthPlix, Practo, etc.), we provide data export and import assistance at no extra charge.
Does it work without internet?
OnePlaceHQ is a cloud-based system that requires internet for full functionality. However, it works well on mobile data (4G/5G) — you don't need a dedicated broadband connection. In areas with poor connectivity, the app caches current queue data so reception staff can issue tokens even during brief outages. WhatsApp messages queue and send automatically when connectivity resumes.
How do patients book appointments?
Patients can book via WhatsApp (click the link you share, choose a slot, confirm via chat), via phone call (staff book on their behalf), or simply walk in for a token. There is no patient app to download — WhatsApp is the booking channel. You can embed a booking link on your clinic's website, Google Business profile, or social media page.
